You can download grant forms in many formats. Paper copies are available on request.
There are three types of grant applications: the General Grant application, the Recognition Grant application and the Public Information and Education (PIE) Grant application. Use the General Grant application for typical grant requests. Use the Recognition Grant form for funding requests less than $15,000. Use the PIE Grant application for funding $2,500 and below.
We recommend that you visit with our staff before submitting an application, especially if you are a first-time applicant. If you have any problems downloading files, please contact us.
