You can download grant forms in many formats. Paper copies are available on request.
There are two types of grant applications: the General Grant application and the Recognition Grant application. Use the General Grant application for typical grant requests. Use the Recognition Grant form for funding requests less than $15,000.
We recommend that you visit with our staff before submitting an application, especially if you are a first-time applicant. If you have any problems downloading files, please contact us.
